How can Eve-Mar Geriatric Care Management assist your patient and help you with their treatment?

Eve-Mar recognizes that first step towards proper medical care is adherence to treatment. In many cases education and communication can assist with helping patients follow through on their treatment plans.  Eve-Mar will assist patients and ultimately you by:

• Making sure patients attend medical appointments with their physicians, occupation therapists, physical therapist, or any other medical professional.
• Helping patients and families understand diagnosis, prognosis, and treatment through continued explanation and education.
• Coordinating treatment services both outside the home and inside the home.
• Taking round the clock calls from the patient.
• Being the first contact for the patient at the time of perceived emergency so that appropriate action can be taken.
• Streamlining medical questions and concerns for patients and families to address with medical professionals during medical or treatment appointments.
• Monitoring treatment and medication compliance.
• Monitoring health changes and providing documentation to facilitate better communication to all involved.
• Diffusing difficult situations.
