How can Eve-Mar Geriatric Care Management assist your residents and help you with their tenancy?

Eve-Mar knows that in order for a resident to remain in their home during their senior years, the home must be safe, comfortable, and appropriate for their changing medical needs.  In many cases basic changes to the environment and coordination of medical care can assist with helping a resident remain in their home for a longer time.  Eve-Mar will assist residents and ultimately you by:

• Completing an assessment of the home environment, and making recommendations to ensure the location if free of hazards for the resident.
• Coordinating modifications to the home with the resident, family, and property manager.
• Obtaining permission from the property manager in making changes that would in any way affect the structure or uniformity of the property.
• Coordinating medical treatment services both outside the home and inside the home as well as monitoring medical treatment and medication compliance.
• Taking round the clock calls from the patient.
• Being the first contact for the patient at the time of perceived emergency so that appropriate action can be taken.
• Helping the resident understand the role of the property manager and other residents so that disturbances can be limited.
• Initiating reminders or completing tasks related to bill pay, correspondence, or record keeping.
• Coordinating communication amongst residents, families, and property professionals.
• Diffusing difficult situations.
