What services will an Eve-Mar Geriatric Care Manager provide?

At Eve-Mar we pride ourselves on providing individual care management to individuals and their families who are experiencing life changing and life challenging issues due to physical and mental diseases brought on by aging.  This could include any of the following services and more:

• Complete home evaluations to assist you in creating a better living environment for you and your elder.
• Coordinate home health care to ensure your elder is cared for and safe.
• Assist in locating appropriate adult day care to meet your elder’s needs.
• Assist in referrals to assisted living which will provide your elder with an appropriate living situation for their changing needs.
• Attend doctor and related appointments to assist you and your elder with understanding of medical services and requirements.
• Consult with other medical professionals involved with your elder such as physical therapist, occupational therapist, respiratory therapist, speech therapists, etc. to ensure your elder is receiving the treatment necessary for their convalescence.
• Coordinate medical appointments and treatment including home visits by medical professionals if needed and available.
• Work with other professionals enlisted to care for elders including attorneys, financial advisors, insurance brokers, property managers, and fiduciaries to name a few.
• Assist in resolving conflict with your elder or other family members who do not understand or accept the needs of your elder.
• Be your eyes and ears for your elder when you cannot due to other responsibilities, geographic differences, or when you just need support.
• Provide caregiver support and education to ensure you are properly taken care of too!
